L’intestin a toujours été considéré comme un simple organe de transport. Il ne se réduit pas cependant à cette seule fonction – certes vitale – d’acheminements des nutriments. Il faut dire que jusqu’aux deux dernières décennies, les processus de digestion et d’absorption des lipides étaient encore mal compris. En outre, on ne reconnaissait à l’intestin grêle aucune propriété modulant l’absorption et, moins encore on n’envisageait qu’il soit à la source de maladies chroniques comme l’athérosclérose. Pourtant, la capacité de transformation des aliments dans la lumière intestinale, la formation des lipoprotéines athérogéniques et l’abondance étonnante de peptides gastro-intestinaux suggèrent que l’épithélium intestinal dispose d’un potentiel considérable d’action au sein d’un grand nombre de voies métaboliques. Cet article de synthèse a pour but de faire le point sur l’avancement remarquable des connaissances dans le domaine de l’absorption des lipides diététiques, en insistant sur le rôle et la contribution des protéines clés pour ce qui se rapporte aux syndromes de malabsorption, ainsi qu’à ceux qui conduisent à l’hyperlipidémie et à l’athérosclérose.
