Abstract
Après avoir résumé les décisions gouvernementales prises depuis vingt ans pour limiter la croissance des dépenses pour les soins de santé, le texte rappelle les critiques de différents documents concernant l'excessive centralisation des décisions et les recommandations pour régionaliser le système de santé et de services sociaux et pour encourager la participation. En dernière partie, la question suivante est analysée : est-il réaliste de vouloir décentraliser dans un univers où le financement des unités régionales n'est pas autonome, mais provient totalement de l'autorité centrale ? Le fait demeure : « He who pays the piper calls the tune ».
