Aerobic Fitness Status and Post-exercise Inflammatory Markers in 19 to 35 Year-old Males

Abstract
Acute physical exercise of various modes results in transient elevations in circulating inflammatory mediators; most significantly interleukin-6 (il-6). Elevated values of circulating il-6 have been reported to enhance fatigue, diminish exercise performance and potentially play a role in overtraining. Increased C-reactive protein (CRP), has also been demonstrated after strenuous exercise. Persistent elevations of il-6 and CRP have also been associated with cardiovascular disease. To date no research has directly compared il-6 and CRP, in trained and untrained subjects in response to a relatively identical exercise challenge. PURPOSE: The purpose of the proposed study is to determine whether baseline and post-exercise concentrations of systemic il-6 and CRP differ between untrained males (U) and endurance trained male cyclists (T). METHODS: In an overnight fasted state after 72 hours without exercise group T (n=13, aged 27+5yrs, VO2max 5.01+.61L) and U (n=12, aged 23+4yrs, VO2max 3.29+.65L) cycled on an upright cycle ergometer for 45 minutes at a wattage corresponding to individual ventilatory threshold (T 247+31 watts, U 133+30 watts). Venous serum il-6 was recorded pre-exercise, 30 minutes post-exercise, and 24 hours post-exercise. Serum CRP was recorded pre-exercise and 24 hours post-exercise. CRP comparisons were made using a 2x2 mixed design ANOVA and il-6 comparisons were made using a 2x3 mixed design ANOVA. Correlations between inflammatory values and subject characteristics were analyzed using the Pearson product-moment correlation. RESULTS: CRP was greater in U than T pre-exercise (1.17+1.43mg/L vs. 0.16+0.25mg/L, P<.05) and post-exercise (1.23+1.23mg/L vs. 0.19+0.20mg/L, P<.05). CRP did not change significantly after exercise in U or T. Il-6 was not significantly different between U and T at any sample point. Il-6 concentrations increased from pre- to 30 minutes post-exercise in U (0.91+.90pg/dl vs. 2.03+1.09pg/dl, P<.05) and T (0.60+0.53pg/dl vs. 1.83+0.74pg/dl, P<.05). Baseline CRP and il-6 were correlated among both groups combined (.59, P<.05). Changes in CRP and il-6 between pre- and post-exercise were not correlated. CONCLUSION: CRP, but not il-6, is significantly reduced at baseline in cycle trained males while post-exercise increases in CRP and il-6 are similar between groups.
