Abstract
HISTORY A 22 year-old left-handed collegiate pitcher presented with a 4-month history of ipsilateral groin and buttock pain. The symptoms started initially following several innings of pitching at the beginning of the season and occurred primarily while pivoting during windup and while pushing off during acceleration. Despite an apparent initial response to rest and nonsteroidals, the pain worsened with continued pitching. He tried stretching and noted reproducible pain with extremes of hip abduction and flexion. He also noted he could not lie flat and when he sits it feels like he's sitting on a rock or something. The patient denied any prior history trauma or pain whatsoever to his lower extremities. PHYSICAL EXAMINATION Gait: slow swing phase on the left, Trendelenberg is negative bilaterally, hip extension-flexion is 0–120 degrees bilaterally, in 90 degrees of hip flexion, external rotation is 15 degrees and symmetric, internal rotation is 30 on the right and 15 on the left, with elicited left hip pain at all extremes of motion. Ober test is positive bilaterally. Strength is 5/5 throughout. He is neurovascularly normal. He is tender to deep palpation in the gluteus medius/piriformis region on the left. DIFFERENTIAL DIAGNOSIS trochanteric bursitis. labral tear. piriformis syndrome. acetabular rim fracture. os acetabuli. femoral stress fracture. bone or soft tissue tumor. psoas hematoma. iliopsoas tendonitis. gluteus medius syndrome. osteitis pubis. TEST AND RESULTS Pelvis and hip anterior-posterior and frog lateral radiographs: posterolateral acetabular lip fracture Additional Tests: Bone scan: increased uptake in region of fragment Magnetic resonance arthrogram: no evidence of labral tear CAT scan 3 months post presentation 7 months post injury: posterolateral acetabular lip fracture with 2mm displacement – callus present FINAL/WORKING DIAGNOSIS acetabular rim stress fracture. TREATMENT AND OUTCOMES Restriction from pitching, running, heavy weights or any impact exercise for eight weeks or until bone healing appears certain. Encourage stationary bike and light resistive exercise. Gradual return to strengthening, running and pitching. Returned to pitching by 3 months after initiating the above conservative measures and was painfree at 18-month follow-up playing professional baseball.
