Depuis plus d’un siècle, la littérature médicale rapporte des cas d’infections virales à l’origine de régressions tumorales. Ce phénomène, aujourd’hui compris, peut être exploité pour la thérapie des cancers. Il fait intervenir des virus dits oncolytiques. Ces virus, sauvages ou mutés par ingénierie génétique, se répliquent préférentiellement dans les cellules malignes. Ils conduisent à la régression tumorale, selon différents mécanismes dont la lyse directe des cellules cancéreuses et la stimulation d’une réponse immunitaire antitumorale. Plusieurs virus oncolytiques sont en phase avancée d’évaluation clinique et pourraient être approuvés prochainement pour le traitement de certaines tumeurs. Ces résultats sont d’ores et déjà prometteurs. Ils peuvent être améliorés, et diverses stratégies génétiques, immunologiques, et pharmacologiques sont à l’étude afin d’augmenter leur efficacité thérapeutique. Cette synthèse retrace l’historique du développement des virus anticancéreux dits oncolytiques, de leur mise en évidence au siècle précédent à leur évaluation clinique actuelle. Elle présente également les principales stratégies qui ont été utilisées, ou qui sont en développement, pour améliorer l’activité oncolytique et la spécificité tumorale de ces agents thérapeutiques.
