ISEE-409 Abstract: In the advanced countries cancer of the prostate has been the most commonly diagnosed cancer among men over the age of fifty-five for over three decades. Age, ethnicity, dietary intake of fat and family history of breast and prostate cancer has been suggested as most important risk factors. However, certain occupational and environmental exposures have also been identified as suspected risk factors of prostate cancer. Environmental exposures to hormonal pesticides during critical phases of prostate development could increase susceptibility to prostate morbidity in andropause years. The aim of this study was to examine the prostate tumor and patient characteristics among farming and non-farming men diagnosed with prostate cancer to test the alternative etiology hypothesis for cancer in farmers. Medical charts of prostate cancer patients diagnosed with prostate cancer between 1985 and 1999 were examined to extract the required information using a pre-designed information extraction forms. A total of 8486 medical charts were reviewed for the fifteen-year period and information on occupation, tumor characteristics, age and prostate specific antigen (PSA) at the time of diagnosis, method of diagnosis, first treatment options and age at death if applicable was extracted. In the study sample 2491 (29%) were farmers, 2361 (27.8%) were non-farmers and no occupational information could be obtained on 3634 (42.8%) patients (unknown). All the men were long term residents of Saskatchewan and included men residing in urban and town centers and in rural areas. Age (mean ± standard deviation) at diagnosis for farmers (72.4±7.4 years) was significantly (p<0.0001) higher compared to non-farmers (69.5±7.9 years) and significantly (p<0.0001) lower compared to the unknown patients. Median PSA levels were significantly higher for farmers (14.5) compared to non-farmers (12.9) and significantly lower compared to the unknown (16.2). Most of the farmers (53.8%) were diagnosed with stage C or D tumor compared to non-farmers (46.2%). The first line of treatments for majority of the farmers (53.9%) was hormonal compared to non-farmers (46.1%) while radical prostatectomy or radiotherapy was the first line of treatment for non-farmers (53.0%) compared to farmers (47.0%). During the study period 1477 (30%) farmers, 1256 (25.5%) non farmers and 2186 (44.4%) unknown patients died. Age (mean ± standard deviation) at death for farmers (78.6±7.8 years) was significantly higher compared to non-farmers (75.8±8.8 years) and significantly lower compared to the unknown patients (81.9±7.9 years). In conclusion farmers were diagnosed with late-stage tumor at a later age and were treated hormonally; they also died at a later age.
