Abstract
A 43-year-old right-handed man was admitted to the hospital after 3 days of progressive back pain with numbness and weakness in the extremities. He was areflexic on admission and the diagnosis of Guillain–Barre syndrome was made. His CSF analysis on admission was unremarkable, but a follow-up nerve conduction study confirmed the diagnosis of acute demyelinating polyneuropathy. His weakness was worse the next day and he was unable to stand without assistance. With informed consent, the patient chose plasmapheresis over IV immunoglobulin because of the fear of infection from receiving pooled blood products. After three sessions of plasmapheresis, his strength had improved to independent ambulation, and his cardiorespiratory status was stable. However, he developed a spiking fever.
