Video-Kapsel-Endoskopie bei Cronkhite-Canada Syndrom

Abstract
Hintergrund: Cronkhite und Canada (1) berichteten 1955 erstmals über zwei Patienten mit multiplen gastrointestinalen Polypen, kombiniert mit Pigmentstörungen der Haut und Atrophie der Finger- und Zehennägel. Bis 2002 wurden weltweit nur 387 weitere Fälle publiziert. Kasuistik: Wir beschreiben den Fall einer 49-jährigen Philippinin. Die Patientin litt an persistierender Diarrhoe und rezidivierender Hämatochezie sowie Gewichtsverlust von 10 Kilogramm in den letzten 6 Monaten vor Einweisung. Bei klinischer Untersuchung zeigte sich ein kompletter Verlust der Finger- und Zehennägel, eine Alopezie und ein subtotaler Verlust des Geschmackssinnes. Familiär gehäuft auftretende gastrointestinale Polyposis bestand nicht. Mikrobiologische Stuhluntersuchungen ergaben keinen Hinweis auf eine infektiöse Genese der Beschwerden. Gastroskopie und Coloskopie zeigten in Magen, Duodenum, terminalen Ileum und im gesamten Colon multiple, diffus verteilte, rötliche, sessile Polypen mit ödematöser Mukosa. Der Ösophagus war nicht betroffen. Das Enteroklysma nach Sellink konnte keine Manifestion im Dünndarm nachweisen. Im Gegensatz dazu fanden sich jedoch in der Videokapselendopskopie (VKE) morphologisch identische Polypen auch im gesamten Dünndarm. Häufigkeit und Ausprägung der Läsionen nahmen in aboraler Richtung ab. Die Dünndarmtransitzeit war mit 2 Stunden 49 Minuten auffällig kurz. Die histologische Aufarbeitung der Proben aus Magen, Duodenum und Colon zeigten eine typische, entzündlich-ödematöse Mukosa und irreguläre Proliferation der Krypten im Colon, vergesellschaftet mit eosinophiler Infiltration in allen Biopsaten. Maligne Veränderungen waren nicht nachweisbar. Unter Berücksichtigung der extraintestinalen Symptome ist dieser histopathologische Befund vereinbar mit dem Cronkhite-Canada Syndrom. Schlussfolgerung: Da eine Adenom-Karzinom Sequenz auch beim Cronkhite-Canada Syndrome vermutet wird (2;3), das Enteroklysma nach Sellink den Befall des Dünndarms jedoch nicht nachweisen konnte, stellt die VKE eine wichtige Erweiterung des intestinalen Screenings bei dieser seltenen Erkrankung dar.
