Abstract
Introduzione.Le ß-lattamasi a spettro esteso (ESBL) rappresentano importanti meccanismi di resistenza nelle Enterobacteriaceae, la cui frequenza varia per area geografica.Riportiamo la prevalenza di ESBL nel 2001-2002, tra le Enterobacteriaceae di 7 microbiologie romane del gruppo di Studio sulle Resistenze Batteriche nelle Infezioni Ospedaliere (SEERBIO).Materiali e Metodi.Identificazioni e antibiogrammi sono stati eseguiti nel 2001-02 su materiali clinici, mediante sistemi automatici, e riuniti in unico database.Gli isolati di Enterobacteriaceae, resistenti al ceftazidime e sensibili a cefoxitina, sono stati considerati produttori di ESBL.È stato escluso Enterobacter, poiché dotato di sistemi vari di resistenza.Risultati.Sono state isolate 10.421 Enterobacteriaceae, appartenenti a 17 generi.Il 13% dei ceppi erano ESBL.I più isolati (89% del totale) sono stati i generi Escherichia (62%, ESBL 5%), Proteus (14%, ESBL 29%) e Klebsiella (13%, ESBL 14%).La prevalenza per area è stata: rianimazioni 32%, medicine 22%, chirurgie 19%, ambulatori 15%.I materiali sono stati urine (48%), espettorato (10%), sangue e t.vaginale (6%), pus (5%).Nelle rianimazioni i materiali sono stati urine (41%), broncoaspirato (34%), sangue (5%) e BAL (4%).I ceppi ESBL resistenti a ciprofloxacina , amoxicillinaacido clavulanico e cotrimoxazolo sono stati rispettivamente 52%, 67% e 37%.Nel 2002 è stata osservata una diminuzione di ESBL: Escherichia -1%, Proteus -8%, Klebsiella -12%.Discussione.Il programma di sorveglianza SENTRY riporta una frequenza di K. pneumoniae ESBL del 45% in America Latina, contro il 7% degli Stati Uniti (con differenze locali).I nostri dati evidenziano una prevalenza del 13%, con un picco del 32% nelle rianimazioni, nonché una variazione annuale.Il genere più frequente è stato Proteus.I ceppi ESBL hanno presentato resistenza anche ad altri farmaci.È stata rilevante la frequenza di ESBL tra i pazienti esterni.Il nostro contributo riguarda circa 3.500 letti di ricovero in Roma e pone le basi per future valutazioni sull'andamento delle resistenze nella capitale.
