Abstract

A haemodialysis patient developed a widespread haemorrhagic macular rash (Fig. 1) 10 days after varicella onset in a nursing contact. The rash was not typical of varicella; herpesviruses were not detected by culture or electron microscopy and the patient had in any case tested varicella‐zoster virus (VZV) seropositive. Biopsy, however, showed intranuclear inclusions suggestive of herpesvirus infection (Fig. 2), VZV DNA was detected by polymerase chain reaction (PCR), and an anamnestic rise of high‐avidity VZV‐specific IgG followed the rash (Fig. 3), indicating secondary varicella. This case demonstrates the utility of PCR in the diagnosis of atypical rashes of possible herpetic aetiology in immunocompromised patients, including haemodialysis patients.
