On February 20, 2006, when John Hoey, editor-in-chief of the Canadian Medical Association Journal (CMAJ), returned to the Ottawa headquarters of the Canadian Medical Association (CMA) after a vacation, his journal was in excellent shape. It ranked as the fifth leading general medical journal in the world, and it received more than 100 original research papers per month, allowing the editors to be highly selective in what they published.
