The London Daily/Nocturnal Hemodialysis Study is the first attempt to obtain comparative data of short high efficiency daily HD [SDH] and long slow HU while the patient sleeps [NH] against conventional 3 x/wk HD [CH]. This is a 3-year observational study to follow 40 patients 10 receiving SDH, 10 NH and 20 CH. The CH patients are cohort controls are matched by age, sex, co-morbidity. All patients are followed for 18+ months and treated at home. Enrollment is complete and all have gone 6 months 7 SDH plus 3 NH for 21 months. This report gives data on calcium metabolism. Initially dialysate concentration was 2.5 mEQ/L [1.25 mmol/L]. There were no differences in pre-dialysis corrected calcium [Ca] levels [2.47±0.24 mmol/L M±SD overall]. Post-dialysis Ca levels were lower in NH eg at 1 month SDH Ca was 2.51 ± 0.16 vs NH Ca of 2.31 ± 0.14 [p < 0.00001]. Pre-dialysis phosphate [P] levels were lower in SDH and NH than CH and with NH in P hinders were discontinued. A rise in alkaline phosphatase was seen in NH patients, which peaked at 18 months [NH 191 IU/L±70 SDH 82±34 CH 80±36 p < 0.002] and similarly with intact parrathyroid hormone levels [iPTH] INH 159 pmol±75 SDH 13.1±10 CH 18±18 p < 0.00001]. At 15 months the dialysate Ca concentration was increased to 3.5 mEQ/L [1.75 pmol/LI for the NH patients. Post-dialysis Ca then rose to 2.57±0.21 and alkaline phosphatase and iPTH subsequently normalized. With NH hut not SDH an elevated dialysate Ca concentration is needed.
