Rehabilitation services after total joint replacement in Ontario, Canada: can ‘prehabilitation’ programs mediate an increasing demand?

Abstract
Total joint replacements (TJR) have emerged as a critical health policy issue. In particular, Canadian demand for these surgeries is forecast to grow annually by 8.7% in the next decade. Although the medical and surgical aspects of TJR have received considerable attention, very little research has explored the impact of increased TJR on the demand for rehabilitation services. In this study, we conducted seven focus group discussions across the province of Ontario (Canada) with multiple stakeholders (n=50) ranging from clinicians and administrators, to policy makers and researchers. Our results indicate that demand for rehabilitation following TJR is rising sharply and that there are three primary factors affecting such demand: (i) increase in the absolute number of TJR surgeries is increasing demand across the continuum of care; (ii) changing profile of clients whereby ‘younger and active’ groups are more willing to undergo surgery, and ‘older and complex’ groups are presenting with increased rates of medical complications and comorbidities; and (iii) widespread use of clinical pathways has increased requirements within the rehabilitation sector, but often without corresponding adjustments in levels of human resources. To align increasing demand with supply in the long term, participants offered strong support for health promotion and prevention programs, but they also highlighted the short-term benefits of implementing ‘prehabilitation’ programs for clients waiting for surgery. Overall, our results indicate that the demand for rehabilitation services after TJR is increasing and that innovative approaches to care delivery are required to align increasing demand with supply. Über die medizinischen und chirurgischen Aspekte von TEP liegen viele Ergebnisse vor, über die Auswirkung einer erhöhten TEP-Rate auf die Nachfrage nach Reha-Einrichtungen jedoch kaum. In der vorliegenden Studie führten wir sieben Fokusgruppen-Diskussionen in der Provinz Ontario (Kanada) mit multiplen Interessenten (n=50) durch, die von Ärzten und Verwaltungsangestellten bis hin zu Entscheidungsträgern und Forschern reichten. Unseren Ergebnissen zufolge steigt die Nachfrage nach Reha-Maßnahmen nach TEP stark an. Ausschlaggebend für eine solche Nachfrage sind drei Hauptfaktoren. Erstens nimmt die Nachfrage über alle Pflegeeinrichtungen hinweg durch den Anstieg der absoluten Zahl der TEP-Eingriffe zu; zweitens ändert sich das Profil des Patientenklientels, insofern als jüngere und aktive“ Gruppen eher bereit sind, sich einer Operation zu unterziehen, und ältere und komplexe“ Gruppen sich mit immer mehr medizinischen Komplikationen und Komorbiditäten in Behandlung begeben; und drittens hat die weit verbreitete Verwendung von Behandlungspfaden den Bedarf innerhalb des Reha-Sektors angehoben, oftmals allerdings ohne entsprechende Personalveränderungen. Um die gestiegene Nachfrage dem Angebot langfristig anpassen zu können, werden die Teilnehmer bzgl. gesundheitsfördernder und präventiver Programme stark unterstützt. Sie haben aber auch die kurzfristigen Nutzen und Vorteile der Realisierung von Reha-Vorprogrammen“ für Patienten hervorgehoben, die operiert werden müssen. Insgesamt zeigen unsere Ergebnisse, dass die Nachfrage nach Reha nach TEP weiterhin steigt und dass innovative Pflegeansätze erforderlich sind, wenn die steigende Nachfrage dem Angebot angepasst werden soll. Las artroplastias totales (AT) se han convertido en un gran problema en el contexto de las políticas sanitarias. Particularmente, en Canadá la necesidad de realizar estos tratamientos quirúrgicos se espera que aumente en un 8.7% cada año durante la próxima década. Aunque los aspectos clínicos y quirúrgicos de las AT han sido ampliamente estudiados, poco se ha investigado sobre las repercusiones para los servicios de rehabilitación del aumento en el número de pacientes que necesitan una AT. En nuestro estudio, realizamos siete discusiones locales de grupo en la provincia de Ontario (Canadá) con varios proveedores de servicios sanitarios (n=50), entre ellos médicos, administradores, encargados de las políticas sanitarias e investigadores. Los resultados obtenidos indican que van aumentando de manera vertiginosa las necesidades de rehabilitación tras una AT, y que son tres los factores que influyen en ello. En primer lugar, el aumento del número de AT realizadas ha aumentado la demanda a todos los niveles de los servicios de salud. En segundo lugar, el cambio experimentado en el tipo de pacientes tratados; es decir, los grupos «más jóvenes y activos» están más dispuestos a recibir el tratamiento quirúrgico y los grupos «menos jóvenes y complejos» presentan una incidencia cada vez más alta de complicaciones médicas y de enfermedades concurrentes. Y en tercer lugar, el uso más extendido de los servicios médicos ha provocado un aumento en la demanda en el sector de la rehabilitación, y en muchas ocasiones sin que se realicen los ajustes pertinentes en los recursos humanos. Con vistas a equiparar a largo plazo la oferta y la creciente demanda, los participantes en el estudio se pronunciaron de manera ferviente a favor de la realización de programas de promoción y prevención sanitarias; aunque resaltaron además los beneficios a corto plazo de establecer programas «prerrehabilitación» para los pacientes en espera de ser operados. En términos generales, nuestros resultados indican que va en aumento la demanda de servicios de rehabilitación tras una AT, y que son necesarias estrategias innovadoras en los servicios sanitarios a fin de equiparar la oferta y la creciente demanda. le remplacement total de hanche (RTH) est en passe de s'imposer comme une question critique en termes de politiques de santé. Tout d'abord, l'augmentation du nombre absolu d'actes chirurgicaux de RTH fait progresser la demande sur tout le spectre des soins médicaux ; deuxièmement, on constate une évolution du profil des clients selon laquelle les groupes «jeunes et actifs» sont plus susceptibles d'opter pour la chirurgie, tandis que les groupes «plus âgés et plus complexes» présentent une augmentation des taux de complications médicales et de co-morbidités; et troisièmement, la généralisation de l'usage des chemins cliniques a entraîné une attente accrue vis-à-vis du secteur de la rééducation, souvent sans qu'il soit procédé aux ajustements correspondants en termes de ressources humaines. Afin d'aligner l'offre sur la demande croissante au long terme, les participants offrent un soutien renforcé à la promotion de la santé et aux programmes de prévention, mais ils mettent également en relief les avantages de la mise en œuvre de programmes de «prééducation» pour les clients en attente d'acte chirurgical. Globalement, nos résultats témoignent de la hausse de la demande pour les services de rééducation après RTH, et des approches novatrices de la prestation de soins sont requises pour aligner l'offre sur cette demande croissante.
