The first task performed by fire fighters on arriving at a fire scene is often stair climbing with hose pack or other equipment. The impact of the added weight on the cardiovascular response is not well understood. PURPOSE: To determine the effects of added weight to the physiological demands and blood pressure response during stair climbing exercise in young adults using the protocol identical to the first 3-min of the Candidate Physical Abilities Test (CPAT). METHODS: Six healthy men participated in this study (age 25.3 ± 2.7 years, height 182.3 ± 1.8 cm, weight 81.9 ± 5.5 kg, and body mass index 24.7 ± 1.9 kg/m2). The tests consisted of: 1) incremental treadmill test to determine maximum oxygen consumption (VO2peak); 2) stair climbing exercise (60 steps/min, 3 min) without extra-weight (NoVest); 3) stair climbing with extra 34 kg vest (Vest). VO2peak, oxygen uptake (VO2), carbon dioxide production (VCO2), minute ventilation (VE) and respiratory exchange ratio (R) were collected with the Cosmed K4b2 portable breath-by-breath system (Cosmed, Italy). Systolic (SBP), diastolic (DBP), and mean arterial pressure (MAP) were determined beat-by-beat (Finometer). Two-way repeated measure ANOVA and Student-Newman-Keuls post hoc test were performed. RESULTS: VO2peak was 54.7 ± 6.8 ml.kg.-1.min-1. For the Vest condition, VO2 (37.3 ± 2.6 ml.kg-1.min-1, VCO2 (3002 ± 47 ml.min-1), VE (85.6 ± 2.4 L.min-1) and respiratory exchange ratio (R, 0.98 ± 0.1) were higher (p<0.05) than NoVest (29.8 ± 1.7 ml.kg-1. min-1, 2115.3 ± 14.9 ml.min-1, 56.3 ± 1.0 L.min-1, and 0.87 ± 0.1, respectively). Furthermore, in the Vest condition, SBP (190.8 ± 2.9 mmHg), DBP (99.0 ± 1.7 mmHg), MAP (130.9 ± 1.6 mmHg), HR (151.1 ± 0.6 bpm), and Q (16.5 ± 1.4 L.min-1) were higher (p<0.05) than corresponding values in NoVest (151.6 ± 1.9 mmHg, 82.7 ± 2.3 mmHg, 113.4 ± 1.9 mmHg, 125.0 ± 0.5 bpm, and 15.1 ± 0.6 L.min-1, respectively). CONCLUSION: Stair climb exercise identical to that used in the CPAT test of fire fighter candidate recruits caused marked elevations in BP. The 34 kg extra weight resulted in higher levels of muscle tension during climbing that augmented the cardiovascular response. These observations in young healthy men raise concern for the BP response that might occur in older fire fighters completing similar tasks in emergency situations.
