Les auto-anticorps dirigés contre le facteur H (FH), principale protéine de régulation de la voie alterne du Complément, sont bien caractérisés chez les patients présentant un syndrome hémolytique et urémique atypique de forme auto-immune (SHU-AI), mais sont moins bien décrits quand ils sont mis en évidence dans un contexte de glomérulopathies à C3 (GC3). Dix-sept patients présentant une GC3 et positifs pour les anticorps anti-FH ont été étudiés. Les données cliniques ont été analysées et les caractéristiques biologiques des IgG anti-FH ont été comparées à celles de 19 patients présentant un SHU-AI. Sept sur dix-sept patients, principalement des enfants, présentaient une double positivité anti-FH et anti-C3 convertase alterne. Une immunoglobuline monoclonale a été retrouvée chez 6/10 patients présentant un anticorps anti-FH isolé. Trois types histologiques étaient représentés : glomérulonéphrites à C3 (10/16), glomérulonéphrite membranoproliférative de type 1 (5/16) et maladie des dépôts denses (1/16). Vingt-neuf pour cent des patients présentaient un dosage sérique de C3 diminué, le dosage sérique du FH étant normal chez tous. La comparaison avec les anticorps anti-FH développés dans un contexte de AI-SHU a révélé que les anti-FH des C3G étaient de plus faible affinité, ne circulaient pas sous forme de complexes immuns, et avaient des sites épitopiques différents, principalement dirigés contre la partie N-terminale de la protéine. Aucun des patients n’était porteur de la délétion CFHR1/R3 classiquement associée au SHU-AI. Une restriction de chaîne des immunoglobulines dans la réactivité anti-FH a été observée chez les patients présentant une C3G et une gammapathie monoclonale (GM) mais pas chez les patients présentant un SHU-AI, y compris adultes. Dans les C3G, les anticorps anti-FH induisent des anomalies fonctionnelles différentes de celles du SHU-AI. Considérant la fréquente association entre la présence isolée d’anticorps anti-FH et une GM chez les patients C3G adultes, une recherche de lymphoprolifération semble recommandée chez ces derniers.
