En dcembre 2003, le gouvernement qubcois sous la gouverne du parti Libral de Jean Charest, adopte sous billon, le projet de loi 25. La Loi sur les agences de dveloppement de rseaux locaux de sant et de services sociaux vise rformer la structure du systme de sant du Qubec. L'ide principale de cette rforme est de crer des Centres de sant et de services sociaux (CSSS), qui, sous la responsabilit des Agences de sant et de services sociaux (ASSS), seront responsables de dvelopper des soins de sant adapts la sant de la population des territoires qu'ils doivent couvrir. L'approche de la sant des populations propose depuis la Commission Rochon est applique plus de 20 ans plus tard. Cette transformation se ralisera toutefois dans le contexte du plan de modernisation de l'tat du gouvernement Charest qui vise alors la rduction de la taille et des dpenses de l'tat. Ceci teintera la rforme du systme de sant qui se verra imposer de manire plus marque qu'auparavant des mesures issues de la nouvelle gestion publique telle que la gestion par rsultats ainsi que la reddition de compte.
