The purpose of this study was to investigate the feasibility and efficacy of an exercise program offered by a community organization on motor performance in individuals with chronic stroke. Twenty hemiparetic individuals at least 6 months after a stroke and discharged from all rehabilitation services enrolled in the study. They took part in an 8-week group program (two times per week) aimed at improving balance, mobility, coordination, walking endurance and strength on the hemiparetic side through various functional exercises. Pre- and post-exercise assessments included the stroke impairment assessment set, the Berg balance scale, the step test, the timed-up-and-go and the 6 min walk. Results showed significant improvements (P<0.008) in all of the above tests, excluding the 6 min walk, after completion of the exercise training. Correlation analyses revealed that the individuals' age and time since injury were poorly related to the improvement in performance. It is concluded that an exercise program offered by a community organization can lead to improvement in motor performance of individuals with chronic stroke. To prevent decline in motor performance, community-based exercise programs should thus be made available for stroke patients discharged from rehabilitation services. Zweck dieser Studie war es, die Machbarkeit und Wirksamkeit eines Übungsprogrammes zu untersuchen, das auf nicht-klinischer Ebene zur Verbesserung der motorischen Leistung bei Patienten mit chronischem Schlaganfall angeboten wird. In die Studie aufgenommen wurden 20 Personen mit Hemiparese, die seit mindestens 6 Monaten an den Folgen eines Schlaganfalles litten und die sämtliche stationären Rehabilitationsgprogramme bereits durchlaufen hatten. Diese nahmen an einem 8-wöchigen Gruppenprogramm teil (2 Sitzungen pro Woche), das darauf abzielte, den Gleichgewichtsinn, die Mobilität, Koordination, Ausdauer beim Gehen und die Muskelstärke auf der von der Hemiparese betroffenen Körperseite durch verschiedene funktionelle Übungen zu verbessern. Die vor und nach den Übungen durchgeführten Bewertungen beinhalteten die umfassende Beurteilung der Schlaganfall-bedingten Einschränkungen, die Anwendung der Berg-Balance-Skala, des Stufentest, des “Timed Up and Go”-Tests sowie 6-minütiges Gehen. Die Ergebnisse zeigten signifikante Verbesserungen (P<0.008) in allen der oben genannten Tests mit Ausnahme des 6-minütigen Gehens nach Abschluss des Übungstrainings. Korrelationsanalysen zeigten, dass das Alter der Betroffenen und die seit dem Schlaganfall verstrichene Zeit kaum einen Einfluss auf die Leistungsbesserung haben. Daraus wird geschlossen, dass ein von einer nicht-klinischen Organisation angebotenes ambulantes Übungsprogramm eine Verbesserung der motorischen Leistung bei Personen mit chronischem Schlaganfall bewirken kann. Daher sollten aus der klinischen Rehabilitationsbehandlung entlassenen Schlaganfallpatienten direkt im Anschluss Trainingsprogramme auf ambulanter Basis angeboten werden, um einen Verlust der motorischen Leistung zu verhindern. El objetivo de este estudio era investigar la viabilidad y la eficacia para la función motora de un programa de ejercicios ofrecido por una organización de la comunidad en personas con ictus crónico. Participaron 20 personas con hemiparesia que habían sufrido un ictus hacía al menos 6 meses y que habían recibido el alta de todos los servicios de rehabilitación. Se incorporaron a un programa en grupo de 8 semanas (dos veces por semana) dirigido a mejorar el equilibrio, la movilidad, la coordinación, la resistencia ambulatoria y la fuerza del lado hemiparésico mediante diversos ejercicios funcionales. Las evaluaciones realizadas antes y después de los ejercicios fueron la evaluación de la afectación por el ictus, la escala del equilibrio de Berg, la prueba del escalón, la prueba cronometrada de levantarse y caminar y la prueba de ambulación en 6 minutos. Los resultados mostraron mejorías significativas (P<0.008) en todas las pruebas anteriores, salvo en la ambulación en 6 minutos, después de realizar el programa de ejercicios. Los análisis de correlación indicaron que la edad de los sujetos y el tiempo transcurrido desde la lesión apenas guardaban relación con la mejoría de la función. Se llegó a la conclusión de que un programa de ejercicios ofrecido por una organización de la comunidad puede mejorar la función motora de las personas con ictus crónico. Para evitar el deterioro de la función motora, es necesario proporcionar programas de ejercicios basados en la comunidad a los pacientes con ictus que ha recibido el alta de los servicios de rehabilitación. Le but de cette étude était de déterminer la faisabilité et les effets d'un programme d'exercices offert par un organisme communautaire sur la performance motrice de personnes atteintes d'un accident vasculaire cérébral (AVC) en phase chronique. Vingt sujets hémiparétiques atteints d'un AVC depuis au moins 6 mois et ne recevant plus de services de réadaptation ont été recrutés pour cette étude. Les sujets ont participé à un programme d'exercices de groupe d'une durée 8 semaines (deux séances par semaine) ayant pour but d'améliorer l'équilibre, la mobilité, la coordination, l'endurance à la marche et la force des membres parétiques par le biais d'exercices de type fonctionnel. Les mesures de performance motrice pré et post entraînement ont été effectuées à l'aide du stroke impairment assessment set, du Berg balance test, du step test, du timed-up-and-go et du 6 min walk. Les résultats ont démontré des améliorations significatives (P<0.008) au niveau de tous les tests à l'exception du 6 min walk à la suite du programme d'exercice. Les analyses de corrélation ont démontré que l'âge des participants et le temps écoulé depuis l'AVC étaient faiblement reliés à l'amélioration de la performance des participants. Cette étude a donc démontré qu'un programme d'exercices offert par un organisme communautaire permet l'amélioration des performances motrices de personnes atteintes d'un AVC en phase chronique. Afin de prévenir le cycle de déconditionnement physique, des programmes d'exercices devraient être offerts dans la communauté aux patients hémiparétiques n'ayant plus accès à des services de réadaptation.
