Abstract
Le virage ambulatoire du réseau de la santé au Québec implique une réorganisation profonde des pratiques. Une étude exploratoire concernant le service de maintien à domicile (MAD) d'un centre local de services communautaures (CLSC) permet d'en décrire certains aspects et d'expliquer comment des infirmières vivent dans ces conditions nouvelles et y réagissent. Outre leur fonction de soignante, ces infirmières sont chargées de cas. Cette fonction de coordination comprend des tâches d'interaction, d'évaluation, de planification, de mise en oeuvre ainsi que de surveillance et de contrôle. Dans un contexte où une logique de réduction des coûts domine, l'ampleur des tâches des infirmières semble fortement sous-estlmée. Leur nouvelle fonction de chargée de cas s'avère une source paradoxale de valorisation, car, malgré leur intérêt, ces responsabilités accrues sont associées à plusieurs difficultés.
