Il sistema sanitario canadese, in cui le diverse province hanno piena responsabilita dell’assistenza sanitaria, offre esempi interessanti. In Ontario per i pazienti oncologici (Cancer Care Ontario) si e passati da un’assistenza frammentata ad un sistema di cure integrato, capace di rispondere ai diversi bisogni dei pazienti, considerati ormai come cronici. Sempre in Ontario infatti sono stati prima introdotti i Family Health Teams, per favorire il coordinamento dell’assistenza, e poi dal 2005 i Local Health Integration Networks, strutture governative locali con il compito di pianificare, coordinare e finanziare servizi specifici. In Quebec invece sono stati creati dei network che prevedono un coordinamento fra il sistema sociale e quello sanitario (Health and social services centres).
