In contributing to the discussion on the safety of remifentanil for obstetric analgesia 1-4, Brampton and Gibson suggest that opioid-induced muscle rigidity was implicated in the adverse events reported 5. Although chest wall rigidity is claimed to be a complication of potent, short-acting opioids such as fentanyl and sufentanil, several authors report that glottic closure, a form of laryngospasm, is the actual problem 6, 7. Since this is a class effect, remifentanil is also implicated. Labour is associated with periods of laryngeal closure, reaching maximal values during intense Valsalva manoeuvres, and we speculate that the periods of increased laryngeal tone during labour may contribute to the risk of opioid-induced glottic closure.
