Abstract

【目的】変形性膝関節症 (以下、 膝OA) に伴う運動機能と痛みに対し、 鍼刺入深度の違いが及ぼす効果を検討した。 【対象】膝OAと診断された患者のうち研究条件に適合した患者26名。 【方法】コンピュータによりA) 浅刺群 (3mm前後の刺入)、 B) 深刺群 (10~20mm刺入) の2群に分類し、 下肢の圧痛点10ケ所に対して10分間の置鍼を行った。 【評価】膝痛の主観的な評価としてvisual analogue scale (VAS) を、 運動機能の客観的な評価としてTimed Up & Go test、 20m歩行時間、 階段昇降時間を、 膝OAのQOL評価としてWestern Ontario and MacMaster Universities osteoarthritis index (WOMAC) をそれぞれ4週ごとに計4回記録した。 【結果】痛みの程度は両群ともに治療前と比較して有意に改善したが (p<0.05)、 客観的な運動機能はすべての測定項目において、 治療前と比較して浅刺群のみ有意に改善した (p<0.05)。 【結語】浅刺は痛みと運動機能を改善しQOL向上に寄与したことから、 膝OAの症状に対する、 より効果的な治療方法である可能性が示唆された。
