Abstract
Résumé L’âge maternel avancé (35 ans ou plus) est un facteur de risque de naissance prématurée, de retard de croissance intra-utérin, de morbidité/mortalité périnatale et de césarienne. Malgré les risques qui croissent exponentiellement avec l’âge maternel, la morbidité et la mortalité maternelles et périnatales demeurent globalement basses. Les femmes donnant naissance à un âge avancé prennent souvent grand soin de leur santé et jouissent fréquemment d’une très bonne qualité de soins, ce qui amoindrit l’effet de l’âge sur le déroulement de leur grossesse et de leur accouchement. Les parturientes très âgées (45 ans ou plus) ont aussi un devenir globalement satisfaisant, peut-être en partie parce qu’elles ont recours au don d’ovocyte. Cependant, le taux de césarienne et la mortalité maternelle sont franchement accrus. L’anesthésiste-réanimateur doit donc rechercher les comorbidités anténatales chez ces patientes et doit être prêt à assurer l’anesthésie pour une césarienne en cours de travail ainsi que la gestion des éventuelles complications aiguës du péripartum.
