Abstract
Trente ans après l'assurance-hospitalisation et vingt ans après l’assurance-maladie, les soins de santé se présentent comme un système provincialisé financé à 80% par la taxation. Plusieurs de ses caractéristiques entraînent une affectation centralisée des ressources. Depuis 1970, les dépenses en ce domaine accusent une tendance à la hausse beaucoup moins prononcée par rapport à une production intérieure brute que pendant les décennies précédentes et connaissent même des périodes de stabilité, grâce aux contrôles gouvernementaux sur la capacité du système. Malgré les récents discours décentralisationistes, la question n'est pas celle du choix entre décentralisation et centralisation —puisque le financement est centralisé—, mais plutôt celle du moyen d'aménager la centralisation qui est sujette à ne répondre qu'aux crises, réelles ou artificielles.
