Success indicators for integrating mental health interventions with community-based rehabilitation projects

Community interventions for people with physical disabilities and for people with mental illness have evolved following similar trajectories, although at different periods of time. This study develops and tests indicators for successful integration of community-based rehabilitation (CBR)-mental health and development (MHD) services. An in-depth study was conducted of two organizations in Sri Lanka and India that successfully integrated CBR and MHD services as well as two organizations in Nepal and Bangladesh, which were planning integration. Interviews and focus groups were used to gather nonconfidential information. The study suggests many benefits of integration and several indicators of readiness: willingness to work with mentally ill people, a basic understanding of the mental health concept and mental illness problems, a match of context and strategy between current CBR activities and proposed MHD activities, stability of basic resources and infrastructure in the organization. A second set of indicators determined the long-term viability of an integrated CBR-MHD approach: ability to strategize and plan a mental health programme, ability to network with stakeholders effectively, ability to make use of resources efficiently. A major finding of the study was the need for training in the practical aspects of integration of mental health interventions with CBR. Tool sets are available that can be used by donors and by local organizations for assessing needs and readiness as well as developing viable strategies for the integration of community-based mental health interventions into existing CBR work. Die vorliegende Studie entwickelt und analysiert Indikatoren einer erfolgreichen Integration von gemeindenahen Reha-Programmen (CBR) und Programmen für die seelische Gesundheit und Entwicklung (MHD). An einer ausführlichen Studie nahmen zwei Organisationen aus Sri Lanka und Indien teil, denen die erfolgreiche Integration der CBR- und MHD-Programme gelungen ist, sowie zwei Organisationen in Nepal and Bangladesch, die eine Integration planten. Ein wichtiges Ergebnis dieser Studie war die Notwendigkeit für eine Schulung in den praktischen Aspekten der Integration der seelischen Gesundheit mit CBR. Spendern und lokalen Organisationen stehen Tools zur Verfügung, mit denen sie nicht nur die Bedürfnisse und die Bereitschaft beurteilen, sondern auch realisierbare Strategien für die Integration von gemeindenahen Interventionen zur seelischen Gesundheit in die bereits bestehende CBR-Arbeit entwickeln können. Los programas de atención local para individuos con discapacidades físicas y para individuos con trastornos mentales han seguido una trayectoria similar, aunque en distintos períodos de tiempo. Este estudio desarrolla y pone a prueba indicadores de la integración exitosa de los servicios locales de rehabilitación y de salud mental y de desarrollo (SLR-SMD). Se realizó un estudio exhaustivo de dos organizaciones en Sri Lanka y en la India que lograron integrar de manera exitosa los servicios de SLR y los de SMD, y dos organizaciones en Nepal y Bangladesh que están planificando el proceso de integración. Para la recolección de información no confidencial se realizaron entrevistas y se estudiaron grupos específicos. El estudio reveló muchos beneficios de tal integración y varios factores pronósticos del estado de preparación de estos servicios: la disposición para trabajar con personas con trastornos mentales, el dominio de conocimientos básicos sobre el concepto de salud mental y sobre los trastornos de la salud mental, la correspondencia contextual y estratégica entre las actividades actuales de los servicios de SLR y las actividades propuestas para los servicios de SMD, la estabilidad de los recursos básicos y la infraestructura de la organización. Un segundo grupo de indicadores determinaron la viabilidad a largo plazo de la integración de los servicios de SLR y los de SMD: la capacidad para planificar un programa de salud mental y elaborar las estrategias de este, la capacidad para trabajar en equipo de manera eficaz con todos los participantes en la implementación de estos servicios, la capacidad para usar de manera eficiente los recursos disponibles. Un resultado importante de este estudio fue la necesidad de adiestramiento sobre los aspectos prácticos de la integración de los servicios de salud mental y los de SLR. Hay disponibles un grupo de herramientas que los donantes y las organizaciones locales pueden utilizar para valorar las necesidades y el grado de preparación, y para diseñar estrategias viables para la integración de los servicios locales de salud mental al trabajo realizado por los SMD. Cette étude a pour objet de développer et de tester des indicateurs d'intégration réussie des services de rééducation-santé mentale et développement (CBR-MHD) au sein de la collectivité. Une étude approfondie a été engagée auprès de deux organisations, au Sri Lanka et en Inde, qui sont parvenues à intégrer avec succès les services CBR et MHD, ainsi qu'auprès de deux organisations du Népal et du Bangladesh qui préparaient leur intégration. L'un des principaux résultats qui ressortent de l'enquête est le besoin de formation aux aspects pratiques de l'intégration des interventions de santé mentale avec la rééducation au sein de la collectivité (CBR).
