Self-actualization and poststroke rehabilitation

Rehabilitation is influenced by the individual's abilities and resources and extent to which they are used. This study explored self-actualization (capacity to maximize use of one's abilities and resources) in 37 individuals attending in-patient rehabilitation following a stroke by (1) comparing the level of self-actualization of the participants to the level of self-actualization of 562 healthy community-dwelling individuals without stroke; (2) assessing changes in the level of self-actualization during rehabilitation (admission vs. discharge); and (3) exploring the relationships between self-actualization and changes in functional independence during rehabilitation, length of stay in the rehabilitation unit and change in living environment at discharge. Self-actualization was assessed with the measure of actualization of potential. The results showed that individuals with stroke had significantly lower self-actualization levels (P<0.001) than community-dwelling individuals without stroke. Self-actualization levels remained stable during rehabilitation (P=0.07). No significant relationships were found between self-actualization and changes in functional independence (P=0.36), length of stay (P=0.82) or change in living environment (P=0.65). This study suggests that many individuals with stroke are poorly actualized and self-actualization levels do not improve during rehabilitation. Further studies are needed to explore the relationship between self-actualization and other outcome measures in poststroke rehabilitation such as depressive symptoms or social participation. Eine Rehabilitation ist abhängig von den Fähigkeiten und Ressourcen der betroffenen Menschen sowie von dem Ausmaß, wie sie eingesetzt und genutzt werden. Die vorliegende Studie untersucht das Maß der Selbstverwirklichung (d.h. die Fähigkeit, das meiste aus seinen Fähigkeiten und Ressourcen zu machen) von 37 Personen, die sich nach einem Schlaganfall einer stationären Reha-Behandlung unterzogen, durch: (1) Vergleich des Maßes der Selbstverwirklichung der Teilnehmer mit dem Maß der Selbstverwirklichung von 562 gesunden Menschen ohne Schlaganfall in der Gemeinde; (2) Bewertung der Veränderungen im Maß der Selbstverwirklichung während der Reha-Behandlung (Aufnahme verglichen mit Entlassung) und (3) Untersuchung der Beziehungen zwischen Selbstverwirklichung und Veränderungen der funktionellen Unabhängigkeit während der Rehabilitation, Aufenthaltsdauer in der Reha-Klinik und Veränderungen im sozialen Umfeld zum Entlassungszeitraum. Die Selbstverwirklichung wurde anhand der Verwirklichung der Leistungsfähigkeit der Personen bewertet. Die Ergebnisse zeigten, dass Schlaganfallpatienten eine wesentlich niedrigere Tendenz der Selbstverwirklichung hatten (P<0.001) als Menschen ohne Schlaganfall in der Gemeinde. Esto se realizó: (1) comparando el nivel de autoactualización de los participantes en el estudio con el de 562 individuos sanos residentes en la misma localidad y quienes no habían sufrido un accidente cerebrovascular; (2) valorando los cambios ocurridos en el grado de autoactualización durante el tratamiento de rehabilitación (comparando los valores obtenidos en el momento del ingreso con los obtenidos al momento del alta); y (3) explorando las relaciones existentes entre la autoactualización y los cambios en la independencia funcional durante la rehabilitación, la duración de la estancia en los servicios de rehabilitación, y los cambios realizados al entorno del lugar de residencia después del alta hospitalaria. Se evaluó la autoactualización a partir de la actualización del potencial. Los resultados mostraron que el nivel de autoactualización de los individuos que sufrieron un accidente cerebrovascular fue significativamente menor (P<0.001) que el de los residentes de la misma localidad que no sufrieron esta afección. El grado de autoactualización se mantuvo invariable durante la rehabilitación (P=0.07). No se hallaron relaciones significativas entre la autoactualización y los cambios obtenidos en relación con la independencia funcional (P=0.36), la duración de la estancia (P=0.82) ni los cambios realizados al entorno del lugar de residencia (P=0.65). Pour mesurer l'actualisation, la Mesure d'actualisation du potentiel (MAP) a été utilisée. Les résultats démontrent que les personnes ayant vécu un AVC présentent un niveau d'actualisation significativement plus faible que celui des personnes âgées autonomes (P<0.001). Le niveau d'actualisation n'a pas varié de fac¸on significative au cours de la réadaptation (P=0.07) et il n'était pas relié significativement à l'amélioration de l'indépendance fonctionnelle (P=0.36), à la durée de la réadaptation (P=0.82) ou à l'orientation au congé (P=0.65). Cette étude suggère que plusieurs individus ayant vécu un AVC sont faiblement actualisés et que cette situation demeure stable malgré la réadaptation. D'autres études sont nécessaires pour explorer davantage les relations enter l'actualisation du potentiel et d'autres variables de résultats liés à la réadaptions comme les sentiments dépressifs ou la participation sociale.
