Abstract
In Quebec, occupational medicine focuses mainly on prevention and aims at improving workplaces. Occupational health services are part of the public health organization. The practice of selecting workers through pre-placement physical examination has been totally left aside and periodic medical surveillance is used only when tests meet the requirements of evidence-based medicine and serve preventive goals. It must be ensured that the course of illnesses can be changed before workers are submitted to screening tests or medical surveillance. The community health approach adopted in Quebec has proved its worth. Nevertheless, after more than 25 years, most workers and workplaces are not benefiting from the health services to which they are entitled; today, less than 25% of workers are able to enjoy every right they thought they had won long ago. Moreover, the political will has disappeared long ago. The development and survival of occupational health services and occupational medicine as part of public health practice are closely related to workers’ ability to have their rights respected and to the will of the nation to fulfill its promises.
