Abstract
L'expérience d'un traitement en comorbidité toxicomanie et santé mentale a permis de documenter l'intervention auprès des personnes présentant un ou des troubles de la personnalité. Cet article décrit le programme intégré dans lequel s'insère la trajectoire de ces personnes. Il fait ressortir la complexité de leur tableau clinique ainsi que les implications des interventions les plus courantes dans leur traitement. Il met en évidence certains constats concernant les moments critiques du parcours de cette clientèle. À l'occasion, des vignettes cliniques illustrent le propos. La réflexion des auteurs amène à une reconsidération des conditions de l'intervention. L'avenue qu'elles proposent est celle d'un traitement différencié, intermittent, mais continu. La possibilité d'un traitement individuel au long cours, pour les clients qui adoptent cette modalité, est également retenue. Cet éventail de possibilités répondrait davantage au caractère chronique des deux troubles.
