Abstract
A 55-year-old married man presented to our service with a 3-year history of erectile dysfunction (ED). Prior to seeing us, he had obtained a prescription from his local general practitioner for four sildenafil (Viagra®) tablets (50 mg) and had found them to be ineffective. Subsequently, he also tried two tadalafil (Cialis®) tablets (20 mg) and vardenafil (Levitra ®[20 mg]) with no improvement in his erection function. His past medical history includes obesity (BMI 32), hypertension (on atenolol), active smoker (20 cigarettes per day) with a 40-pack year history and diabetes (diet-controlled). He reported that he is unable to achieve and maintain his erection in nearly all circumstances on all occasions with his wife, whom he describes as supportive. He denied previous penile trauma or any deformity, and believes that his erections have become progressively less rigid over the past 3 years. He denied any sensory changes on the shaft or glans. His sexual interest remains strong but even with self-stimulation he is unable to gain adequate rigidity, but reported that his ejaculation and orgasm remain intact.
