Après avoir brièvement rappelé les principales caractéristiques de l'évolution récente du champ de la santé au Québec, l'article situe historiquement la création des D.S.C., qui se fit à la fois en continuité et en rupture avec les anciennes structures de santé publique (unités sanitaires et services municipaux de santé). Quelques données descriptives permettent ensuite de voir où sont situés géographiquement les D.S.C., de quels budgets ils disposent, quel genre de personnel ils emploient et quels services concrets ils offrent à la population. Certaines questions de nature plus analytique sont ensuite posées : les D.S.C. peuvent-ils réellement participer à améliorer l'état de santé de la population ou ne sont-ils qu'un nouveau terrain où des professionnels acquièrent ressources et prestige? Quelle est la conséquence pour les D.S.C. de la création d'une spécialité médicale en santé communautaire? Quelle est la nature des relations entretenues par les D.S.C. avec les organismes communautaires? En tant que protagonistes majeurs des réformes affectant la santé et la sécurité en milieu de travail, dans quel sens risquent-ils d'influencer les services offerts aux travailleurs? L'avenir de ces organismes fort originaux issus de la réforme du réseau des affaires sociales est ensuite discuté. Une bibliographie détaillée sur les D.S.C. complète l'étude.
