Résumé Rien ne bouge sans changement interne au sujet. Ni efforts de volonté, ni mises en demeure, ni exigence politico-administrative de résultat rapide, ni protocoles standards ne peuvent dispenser de la prise en considération de la clinique de chaque cas ou, dans le cadre général des addictions, de la spécificité de l’objet choisi. La capacité de « s’arrêter » est potentielle et se réalise à son heure. La décision est le fruit d’une évolution latente, parfois longue. Quatre vignettes cliniques concernant des patients alcooliques illustrent l’apparition du processus décisif qu’est l’effet d’après-coup.
