We observed a patient who developed severe autoimmune neutropenia, thrombocytopenia, and membranoproliferative glomerulonephritis with severe acute renal failure during the course of an apparent viral illness. Viral inclusion bodies were found on urinary cytopathology and tubular cells in the urine as well as in the renal parenchyma on a renal biopsy. However, all viral cultures and serology were negative or normal. The renal failure improved with rehydration and high-dose intravenous IgG. The neutropenia and thrombocytopenia responded only briefly but were brought into prolonged remission by oral administration of prednisone.
