Abstract
The role of antibiotics in the treatment of inflammatory bowel disease (IBD) has been controversial for the past several decades. To resolve the debate of the utility of antibiotics in IBD it is imperative to explore their role in different clinical settings including treatment of active luminal Crohn's disease (CD), treatment of perianal complications of CD, prevention of postoperative recurrence in CD, treatment of pouchitis, and the treatment of toxic complications of IBD. The evidence for the use of antibiotics in these roles will be discussed in this chapter. There are several studies looking at the use of antibiotics as primary therapy for luminal CD. Unfortunately, the majority of these are observational, uncontrolled studies or lack sufficient power to truly detect important differences. Many antibiotics have been used; however, ciprofloxacin and metronidazole are the most commonly used in clinical trials as well as in clinical practice.
