Evidence for the Presence of a Functional TSH-Receptor in Retroocular Fibroblasts from Patients with Graves' Ophthalmopathy

Konzepte zur Pathogenese des Morbus Basedow müssen sich an der klinischen Trias aus Hyperthy-reose, endokriner Orbitopathie und prätibialem Myxödem orientieren. Der Nachweis eines funktionell bedeutsamen TSH-Rezeptors auf retroorbitalen und prätibialen Fibroblasten könnte die Hypothese eines gemeinsamen Antigens bzw. zellulären Aktivierungsmechanismus von Schilddrüse und extra-thyreoidalen Manifestationen erhärten. Die vorgestellten Studien charakterisieren die funktioneilen Effekte gereinigter G-Immunglobuline mit hoher TSH-Rezeptor-stimulierender Aktivität (TSI) auf zwei im Schilddrüsen-, Retroorbital- und Präti-bialgewebe nachgewiesene Proteine. Die stimulierenden Effekte dieser IgG-Präparationen auf die Expression des 72-kDa-Streßproteins, die Expression des Adhäsionsrezeptors intercellular adhesion molecule-1 (ICAM-1) sowie die ICAM-1-vermittelte Lymphozytenadhäsion sind dosisabhängig, spezifisch für TSI-positive IgGs und selektiv in retroorbitalen Fibroblasten von Patienten mit endokriner Orbitopathie nachweisbar. Unsere Befunde sprechen für die Existenz eines aktivierbaren TSH-Rezeptors im Retroorbitalgewebe, von dessen weiterer funktioneller und immunologischer Charakterisierung Aufschlüsse über seine Rolle in der Pathogenese der Basedow-Trias zu erwarten sind.
