Abstract
La physiopathologie des douleurs pelvi-périnéales est complexe. Sa compréhension nécessite des compétences anatomiques et neurophysiologiques dans ce domaine. Il faut cesser de raisonner en termes d’organe, mais plutôt s’attarder à comprendre et à conceptualiser les mécanismes à la base de la genèse et de la régulation de la douleur au niveau local, régional, spinal et central. Parmi les syndromes les plus souvent rencontrés, on retrouve la cystite interstitielle, les différents types de douleurs vulvaires, la douleur pelvienne chez l’homme, les douleurs musculosquelettiques, le syndrome du releveur et la proctalgia fugax. On ne saurait passer sous silence le syndrome du côlon irritable (SCI), souvent apparenté à ces pathologies et cité lors de l’anamnèse. Toutes ces douleurs ont en commun une hypersensibilisation périphérique, voire centrale comme conséquence d’un bombardement chronique surtout nociceptif. Les hypothèses de travail, pour expliquer ces douleurs, appellent des facteurs inflammatoires, immunologiques, génétiques, neurologiques, neuro-endocriniens et psychologiques, alors que l’étiologie est le plus souvent inconnue.
