Commentary: the ss469415590 <scp>SNP</scp> – an <scp>IL</scp>28B marker looking for a home

Abstract
The identification of single-nucleotide polymorphisms (SNP) in the IL28B (or IFNL3) gene through genome-wide studies was a revelation in the hepatitis C virus (HCV) arena. Differences in IL28B phenotypes partially explained the previously poorly understood differences in response to interferon-based therapy in different ethnicities with HCV.1 A number of different SNPs have been identified including rs12979860,2 rs80999173, 4 and rs129802754 with the most frequently used being the rs12979860 SNP (CC/CT/TT).5 Covolo and colleagues6 report their findings regarding response to interferon-based therapy with the ss469415590 SNP of the IL28B phenotype in an Italian, predominantly Caucasian, population. They found that the ss469415590 SNP had similar predictive value in patients with HCV genotype 1 as compared to both rs12980275 and rs12979860, but was superior to rs8099917, which parallels the Austrian experience.7 Similar to the other IL28 SNPs, ss469415590 had no significant predictive value for genotype 2 and 3 patients. Although interferon-free treatment is just on the horizon, reports of interferon's death are likely greatly exaggerated. Given the probable high cost of interferon-free treatment, interferon will still have a role in treatment of HCV and subsequently a continued role for IL28B testing in genotype 1 patients. The ss469415590 SNP's role currently remains unclear due to its limited study outside of a Caucasian population and lack of significant superiority to the rs12979860 SNP, outside of possibly the HCV/HIV coinfected population.8 Given the overlap of the odds ratios for all the IL28B SNPs in the advanced fibrosis subset, it is unlikely that ss469415590 will demonstrate significantly better predictive power in this population. Further investigations in non-Caucasian populations will be needed to improve global generalisability, given that many of the countries where interferon will remain as the primary treatment for economic reasons are predominantly non-Caucasian. Until then, ss469415590 remains an IL28B marker looking for a home. Declaration of personal and funding interests: None.
