Abstract
Résumé Cet article analyse la nature des services de santé ainsi que l'état de santé de la population en fonction de l'économie politique où ils s'insèrent. L'auteur étudie, ici, le cas américain, le replaçant dans une analyse de la lutte plus générale pour les bénéfices issus du contrôle et de la propriété des moyens de production des soins, dans le contexte d'une société capitaliste avancée. Il passe d'abord en revue les antécédents historiques des États-Unis. Ensuite il considère la lutte sous trois aspects: la lutte pour le contrôle du travail, la lutte pour façonner l'organisation des services, et la lutte à propos du rôle de l'État dans le domaine de la santé. Utilisant le concept de l'autonomie relative de l'État dans les sociétés capitalistes avancées (Poulantzas), il conclut par un résumé théorique, qui jauge deux possibilités futures: un système public d'assurance-maladie et un système nationalisé de services de santé.
