Les auteurs analysent la dynamique sociopolitique des organisations publiques centralisées, plus précisément celle du système de la santé et des services sociaux du Québec, à travers deux cas de fusion d'hôpitaux imposée par le gouvernement: le premier sur la haute Côte-Nord et le second dans la Beauce. Les résultats de cette étude montrent que le pouvoir des acteurs centraux sur ceux de la périphérie (administrateurs et gestionnaires d'établissements, professionnels, usagers) n'est toutefois pas sans limite. Lorsque les acteurs locaux parviennent à déplacer dans le champ politique l'enjeu d'une décision administrative, ils peuvent contrer ou même inverser le rapport de dépendance qui les lie au centre. Les deux cas indiquent que la cohésion des acteurs locaux et l'engagement des médecins dans ces coalitions jouent un rôle déterminant dans le succès de ces dernières.
