Abstract
We thank Mandegar et al. [1] for their interest in our article entitled ‘Intra-atrial implantation of a mitral valve prosthesis in a heavily calcified mitral annulus’ [2]. To avoid the risk of dehiscence, they describe a technique whereby the mitral valve (MV) prosthesis is surrounded by a Dacron patch that is then sutured to the atrial wall. This technique of ‘Dacron collar’ was initially described by Nataf et al. [3] who reported an operative mortality of 36% with a valve dehiscence noted in up to 18% of their patients. It is important, therefore, to emphasise that even with the use of the Dacron patch, the risk of dehiscence is still present. For this reason, this technique should only be used in extremely rare cases, only as a life-saving procedure in high-operative-risk patients when no other traditional valve implantation techniques are possible. A close follow-up of these patients with serial echocardiograms is also recommended.
