Abstract
Zusammenfassung Das akute Kompartmentsyndrom wird durch die inkongruente Beziehung zwischen dem Volumen des Kompartments und seinem Inhalt ausgelöst. Dadurch entstehen erhöhte Drücke in Ruhe und bei Belastung, die die Mikrozirkulation unterbinden und damit zum Untergang der intrakompartimentären Strukturen führen. Ein chronisches Kompartmentsyndrom war bisher nur in der Sport- und in der Wehrmedizin bekannt. Die Krankheit betrifft in der überwiegenden Zahl der Fälle das anteriore Kompartment und die Muskelgruppe der Dorsalflexoren, selten die laterale Muskelloge. Durch spektroskopische Untersuchungen ließ sich eine vemehrte Desoxygenierung des Muskelgewebes nachweisen, es kommt aber nicht zu Muskelgewebsnekrosen. Im Verlaufe von schweren chronischen Venenkrankheiten entwickelt sich ebenfalls ein chronisches Kompartmentsyndrom, das sich aber grundlegend unterscheidet. Es ist immer in dorsalen Kompartments lokalisiert. Die narbige Destruktion der Fascia cruris wirkt sich bei jedem Schritt des Patienten auf das Muskelgewebe aus. Die Folge sind schwere Veränderungen im Sinne einer chronischen Ischämie mit Nekrosen und Glykogenverarmung der Muskulatur. Zur Definition des Krankheitsbildes sind weitere Untersuchungen notwendig, vor allem intrakompartimentäre Druckmessungen unter dynamischen und standardisierten Bedingungen. Es wird vorgeschlagen, die zwei Formen eines chronischen exertionellen Kompartmentsyndroms und eines chronischen venösen Kompartmentsyndroms auch verbal zu unterscheiden.
