Cet article expose les fondements de l’évaluation orientée sur l’intégration des services, et montre en quoi elle structure la conception d’une intervention collective plus cohérente impliquant l’ensemble des ressources nécessaires au maintien de l’autonomie de la personne âgée. Cette forme d’évaluation participe d’un changement important dans la fonction de toute évaluation clinique, en favorisant une approche centrée sur les besoins de l’usager plutôt que sur ceux des systèmes ou des professionnels. Pour être efficiente, l’évaluation à visée intégratrice doit favoriser l’émergence d’un interlangage entre des pratiques professionnelles interdisciplinaires, inter-organisationnelles ou intersectorielles.
