Abstract
L'utilisation répandue des drogues neuroleptiques en psychiatrie a amené une vague de maladies iatrogéniques et leurs conséquences socio-légales, phénomènes qui restent incomplètement examinés par les professions impliquées. Les intervenants sociaux manifestent une presque totale ignorance de ces problématiques. L'auteur résume une partie de l'évidence récente concernant les effets thérapeutiques et toxiques des neuroleptiques. Les effets thérapeutiques semblent avoir été exagérés, et ceci au détriment d'une meilleure reconnaissance des effets toxiques. En particulier, les symptômes extrapyramidaux, bien que largement répandus, passent souvent inaperçus ou sont alors confondus avec les symptômes de désordres psychiatriques. Le refus ou l'incapacité d'envisager ce problème de la perspective du patient fait perpétuer des souffrances inutiles. L'auteur discute des enjeux professionnels et politiques qui sous-tendent l'utilisation des neuroleptiques et du besoin pressant de réexaminer objectivement ses justifications. Des pistes d'intervention et de recherche appliquée et fondamentale en psychopharmacologie sociale sont proposées.
