Abstract
La difficulté d'accès au dossier du patient, le dédoublement des efforts et les problèmes de communication au sein de l'infrastructure médicale font partie des grands problèmes qui nuisent à la qualité des soins et à l'efficacité du système de santé canadien. De plus, les patients demandent d'avoir accès plus rapidement aux bons services, d'obtenir de meilleurs résultats cliniques et de participer davantage à la prise en charge de leurs propres soins. Le présent article expose un cadre de soins axés sur le patient afin d'aider les dispensateurs de soins à intervenir mieux et plus rapidement auprès des patients, par des plans thérapeutiques fondés sur leurs antécédents médicaux détaillés. RelayHealth propose un cadre pour coordonner les soins axés sur le patient et intégrer pleinement les interactions entre le patient et le dispensateur de soins, à tous les points de service du continuum de la santé.
