ent presenter au moins l’un des cinq diagnostics principaux suivants selon la classification internationale des maladies version 10 (CIM-10) : maladie coronarienne, pneumonie acquise en milieu communautaire, maladie pulmonaire obstructive chronique, insuffisance cardiaque ou diabete de type 2. Le choix de ces affections est motive par leur frequence elevee, leur taux eleve de readmission et la presence de donnees probantes relatives a la pharmacotherapie choisie. Les patients etaient exclus de l’etude si leur duree d’hospitalisation etait inferieure ou egale a deux jours, s’ils avaient recu un diagnostic de cancer a un stade avance, s’ils provenaient d’une autre unite de soins (c.-a-d. transfert entre etablissements) ou s’ils residaient a l’exterieur de la region administrative.
