Abstract
Le but du suivi systématique clinique décrit dans cet article était d'améliorer l'état de santé et la qualité de vie des personnes itinérantes souffrant de troubles mentaux graves et persistants et de les aider à utiliser les services de santé et de bien-être de façon appropriée. C'est au moyen d'une étude descriptive de 12 mois auprès de 14 personnes itinérantes que les données relatives aux interventions du case manager et à leur évaluation ont été recueillies auprès de divers intervenants du CLSC des Faubourgs, de ressources communautaires et d'établissements de santé. L'expérience s'est avérée positive malgré sa courte durée et les ressources limitées qui y furent affectées. Des recommandations sont présentées en vue d'une action concertée de tous les intervenants auprès de cette population.
