Abstract
Resume Objectif : Analyser les differentes modalites de la chimioembolisation, les indications de ce traitement ainsi que les grands principes qui sous-tendent son utilisation. Sources de donnees : Les donnees proviennent d’une recherche menee sur PubMed ainsi qu’Embase a l’aide des termes « chemoembolization », « hepatocellular cancer », « colorectal cancer », « melanoma ». Le document du Comite de l’evolution des pratiques en oncologie sur les therapies transarterielles pour le traitement du carcinome hepatocellulaire en a egalement fourni ainsi que certaines references d’articles de revue. Certaines proviennent aussi de la consultation de quelques manuels de medecine generale et d’oncologie medicale. Selection des etudes et extraction des donnees : La majorite des donnees presentees proviennent de revues de traitements, de revues systematiques, de meta-analyses ou d’etudes cliniques. Analyse des donnees : On appelle chimioembolisation l’administration de la chimiotherapie par voie transarterielle directement dans une branche de l’artere hepatique irriguant la tumeur, puis l’embolisation de celle-ci. La chimioembolisation est principalement utilisee en traitement palliatif pour certains patients selectionnes presentant un carcinome hepatocellulaire. Certaines etudes montrent aussi son efficacite en traitement palliatif de metastases hepatiques du cancer colorectal, du melanome uveal ou de la tumeur neuroendocrine pancreatique. Conclusion : La chimioembolisation demeure un traitement palliatif a visee restreinte, administre seulement dans quelques centres specialises au Quebec. Le pharmacien a un role a jouer dans la preparation des medicaments et le suivi des patients traites. Abstract Objective: To identify the different chemoembolization methods, the indications for such treatment, and the main principles underlying its use. Data sources: A search was conducted in PubMed and Embase using the keywords “chemoembolization,” “hepatocellular cancer,” “colorectal cancer” and “melanoma.” The CEPO document on transarterial therapies in the treatment of hepatocellular cancer was used, as well as references listed in review articles. General medicine and medical oncology textbooks were also consulted. Study selection and data extraction: Most of the data presented are from treatment reviews, systematic reviews, metaanalyses and clinical studies. Data analysis: Chemoembolization consists in administering chemotherapy via the transarterial route directly into a branch of the hepatic artery feeding the tumour and then embolizing this vessel. Chemoembolization is used mainly as palliative treatment in certain selected patients with hepatocellular cancer. Some studies have also shown its efficacy as palliative treatment for liver metastases from colorectal cancer, uveal melanoma or a pancreatic neuroendocrine tumour. Conclusion: Chemoembolization is a palliative treatment with a narrow scope of application administered only at a few specialized centres in Quebec. The pharmacist has a role to play in preparation of the treatment and in monitoring these patients.
