Le modèle de Système Intégré pour Personnes Agées fragiles (SIPA) entendait offrir la gamme entière de services médicaux et socio-sanitaires à une population âgée fragile d'un territoire défini. SIPA vise à assurer aux organisations locales responsables de la santé et de la prestation des services sociaux toute la flexibilité requise pour répondre aux besoins d'une population âgée fragile d'un territoire. Les SIPA locaux reçoivent les sommes nécessaires à l’ensemble des prestations. Ils assument la responsabilité de la gestion financière et clinique de la prestation des services, mettent sur pied les équipes multidisciplinaires pour la prestation de services de proximité et s'entendent sur une base contractuelle, pour les autres types de services, avec des établissements hospitaliers, d'hébergement médicalisé et autres. La capacité de gestion des SIPA doit être suffisante pour identifier les besoins des personnes âgées du territoire, assurer le fonctionnement en multidisciplinarité des équipes de prestataires, gérer les relations contractuelles avec les établissements et administrer les fonds dont ils sont fiduciaires. Ces responsabilités exigent des équipes locales fortes et compétentes, capables d'affronter les risques financiers et cliniques inhérents aux services qu'ils procurent. Il n’y aura pas de territorialisation possible si ces exigences fonctionnelles ne sont pas intégralement respectées.
