Abstract
Reduced physical activity represents one of the most significant changes in lifestyle of the 20th century but the contribution of exercise to the prevention and treatment of obesity is still seen as trivial by many health professionals. The exercise tolerance of obese people is so low that the level of physical activity they can sustain makes a negligible contribution to overall energy expenditure. Energy balance is linked to macronutrient balance but macronutrients are regulated independently and fat balance shows marked variations. The exercise-trained individual can maintain a reduced level of adiposity because of greater sensitivity and performance of the mechanisms that regulate fat balance. Diet composition is an important determinant of the potential of exercise to induce a negative energy balance. Combining exercise and a reduced dietary fat intake should favour spontaneous body weight loss in obese individuals but adipose tissue-related regulatory factors may favour restabilization of body weight over time.
