The article by Sahoo et al 1 in this issue of the Journal reports an analysis of cytokeratin (CK) 19 immunoreactivity in thyroid nodules and questions its value in the diagnosis of papillary thyroid carcinoma. The controversy raised by this article is only one of many similar issues of major importance to the current practice of pathology. As technology advances, pathologists can apply new methods to assist in making an accurate diagnosis that will ensure appropriate patient management. We have seen waves of new technologies that have affected our practice: electron microscopy, immunohistochemistry, in situ hybridization, and other new molecular diagnostics. The results of these various ancillary techniques are evaluated for their contribution to diagnosis. When we believe that they offer objective scientific evidence that supports the strength of our interpretation, they can be extremely useful tools.
