En Afrique, le traitement du sida par les antirétroviraux est soumis en premier lieu à des enjeux économiques qui réservent l’accès aux médicaments à une infime minorité de personnes. Quels sont les effets culturels de la disponibilité de ces traitements? Les antirétroviraux ne font pas seulement l’objet d’une « indigénisation » décrite à propos d’autres médicaments ; ils sont soumis à une expérience des patients qui, dépassant les limites de l’observance, conduit ces derniers à modifier leurs relations sociales ; ils constituent des révélateurs de « politiques de la vie » antagonistes du global au local en suscitant des catégorisations dans l’accès aux médicaments ; ils polarisent le système médical et conduisent à redéfinir les légitimités notamment aux articulations entre secteurs biomédical professionnel et secteurs traditionnel et populaire. L’impact de ces traitements en termes de modifications des représentations, des pratiques et des rapports sociaux, dépasse largement le secteur biomédical, car ils suscitent la production de médicaments néo-traditionnels dont la légitimité est construite à partir d’emprunts à la biomédecine, sur les terrains laissés vacants par celle-ci dans le champ de l’identité et de l’accessibilité, dans un processus favorisé par la globalisation.
