Abstract
Le développement et la mise en œuvre d'un réseau régional qui assure la prestation de services universels et uniformes aux personnes âgées fragiles vivant dans le sud-ouest de l'Ontario sont décrits. Grâce à l'engagement continu des intervenants, on a défini et opérationnalisé des objectifs de réseau clairs. L'engagement des intervenants envers l'intégration de l'expertise et des services spécialisés, une politique publique fondée sur des données probantes et les cycles d'évaluation itératifs ont été des facteurs de succès clés du réseau.
